Insurance
I accept many major health insurance plans, and am an in-network provider with many. While your health insurance may cover medical expenses, in some cases your insurer has contracted out mental health coverage with another company. Please call to check on these benefits, and clarify the provider network utilized.
Insurances accepted: Aetna, Anthem/Blue Cross Blue Shield PPO & POS, CIGNA, GEHA, Multiplan, PHCS, United Healthcare/Optum, UMR, Carelon Behavioral Health, Kaiser Permanente
(This may not be a complete list since there are subsidiaries of these companies operating under different names).
When you call your insurance company, ask these important questions:
- What is the name of my provider network?
- Do I have a deductible (an out-of-pocket) that must be met prior to payment of claims?
- Do I have an HSA (Health Savings Account) that pays some of my medical expenses prior to meeting my deductible?
- If I have an HSA, does this automatically pay the provider when a claim is submitted?
- If I have an HSA, do I have checks or a debit card that is used for payments?
- If I have an HSA, do I need to submit a receipt for reimbursement of medical expenses paid out of pocket?
- Do I have a Flex Spending Account (often offered instead of an HSA) – how does this work?
- What is my copay for mental health services?
- Are mental health services subject to the deductible?
- Are there limits on the number of visits, or frequency of appointments?
Out of Network Benefits
If I am not an in network provider with your insurance plan, I can submit claims; however, you will need to pay my fees upfront (in accordance with the rates listed above). In these cases, your insurer will reimburse you based on your out-of-network benefits.
EAP: Employee Assistance Program
I am on the panels of many EAP programs. If you would like to use your EAP benefits, you will need to contact the program first to request an authorization to come see me. Please note I participate in approximately ten different programs so it is important to obtain the specific name of your program, or the EAP company that offers these services. Simply stating you were referred by your “EAP” program, or this is offered through my employer (name of the company) does not provide sufficient information to identify the payer.
You will need to obtain an authorization # at the time you call; this is needed for billing, and will also confirm that you are registered in their system. Inquire about the number of visits covered, and if a certain number are approved initially that may be authorized later if, and when the provider submits additional requested information.
These are important questions to get answers to, so there will be no surprises. Insurance companies state a disclaimer when disclosing benefits – this is a statement of coverage; however, it is not a guarantee of payment. You need to be aware that ultimately you are financially responsible should there be a difference in quoted coverage and actual payment for services. (This is also covered in the disclosure statement you receive with administrative paperwork prior to your first appointment).
Payment
I accept payment on Zelle and Venmo as well as cash and check.
Cancellation Policy
If you are unable to attend a session you are responsible for providing notice in accordance with the following policy.
Appointments scheduled on Tuesday through Friday require notice of at least 48 hours if you need to cancel, or reschedule. Appointments scheduled on Mondays must be cancelled no later than 9 am on the prior Friday to avoid being charged a late cancellation fee. Any session scheduled that is not attended or cancelled per terms of Fee Agreement will be charged the full fee for the session. Credit card information may be kept on file for these purposes. Please note the fee in these cases is the actual charge for the session, and not the amount of your copay.
